Personalized Plan of Care



The Coleman Adult Day Nursing staff, along with family members, develop an individualized plan of care on how a client will be cared for while attending Coleman Adult Day Services. This plan addresses medical needs that are communicated for the client’s physician, each physical or cognitive impairment, as well as psychosocial and recreational interests.

Achievable goals are established to empower the client to remain as independent as possible and maintain the highest quality of life. These plans are reviewed by all Coleman Adult Day staff to insure continuity of care. A care plan meeting is held with the client, Coleman nursing staff and family members at least every six months or more frequently if needed, and goals are monitored and adjusted routinely.
